Donations made to the Ukrainian American Youth Caver Exchange Foundation, a Project of the National Speleological Society, should be mailed to "UAYCEF", C/O Chris Nicola, 2446 43 Street, Astoria, NY 11103. In making such donations all checks should be made payable to "UAYCEF". In the case of cash donations the below form should be completed, and maintained as a receipt upon securing the signature of a UAYCEF Board member.
